The baby should wear the necklace to have the amber close by but it should be short enough that the baby cannot put the necklace into her/his mouth. Amber is quite soft. The first teeth could crack the beads and you don't want her/him swallowing the beads. The string is knotted between each bead so that should the necklace rip (which it should if necessary) not all the beads will be scattered and lost.
